myPlace Health is seeking a passionate Care Team Specialist to lead an Interdisciplinary Team (IDT) and ensure high-quality care for our participants. This role is crucial for facilitating team meetings and ensuring that care plans align with the values and preferences of each participant.

Responsabilités

• Partner with IDT to align care with participant's goals.
• Facilitate Care Team Meetings and ensure documentation accuracy.
• Serve as an EMR champion and help with troubleshooting.
• Monitor completion of care plans and remove barriers.
