Job summary

BoldAge PACE is seeking a Care Team Manager to lead the IDT, coordinate care, and ensure high-quality geriatric services in Oceanport, NJ. The role focuses on team supervision, meeting facilitation, and developing comprehensive care plans in collaboration with all disciplines.

The ideal candidate has 5+ years in health care, 3+ years in management, and preferably an RN license. The position offers strong compensation and benefits within a supportive, mission-driven environment.

Qualifications

• Minimum of 5 years working in a health care setting.
• 3 years in healthcare management (geriatric setting preferred).
• RN license preferred.
• Experience coordinating interdisciplinary teams.

Responsibilities

• Manage the IDT, including scheduling and meeting facilitation.
• Develop and implement participant care plans with interdisciplinary input.
• Ensure compliance with quality standards and regulatory requirements.
• Provide leadership and guidance to IDT members.
• Monitor participant progress and communicate changes effectively.
• Maintain documentation and reporting for CMS/PACE.
