AltaMed is seeking a Social Services Coordinator within the PACE program to coordinate essential services and enhance participant satisfaction. You will handle questions, complaints, and disenrollments while ensuring timely Medi-Cal/Medicare resolutions.

The role involves on-site surveys and cross-team collaboration to meet program goals and improve overall participant experience. Preferred candidates hold a bachelor’s degree in social work or a related field, 2+ years in customer service or

Qualifications

• Bachelor's degree in social work or health/human services required.
• At least 2 years of customer service or case management experience, preferably in healthcare.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits, including applications and renewals.
• BLS/First Aid certification required.
• PACE or other healthcare setting experience with frail or elderly populations preferred.
• Bilingual abilities (Spanish/English, or Chinese/English, or other) strongly preferred.

Responsibilities

• Coordinate PACE services and promote participant satisfaction and retention.
• Address participant and family service questions, requests, complaints, appeals, and incident reporting.
• Ensure Medi-Cal and Medicare issues are resolved via established workflows.
• Process involuntary disenrollments and facilitate on-site satisfaction surveys.
• Engage in center-wide participant experience activities and link to community resources.
• Collaborate with MSWs and interdisciplinary teams on case management goals.
