AltaMed in Pomona, CA is seeking a Social Worker to provide a psychosocial perspective in the PACE program, including evaluation, care planning, coordination of care and disenrollment. Interventions may include individual and group modalities, family contacts, education, and leveraging social determinants of health to meet participant needs.

The MSW must have at least 2 years in geriatric case management or mental health, with BLS certification or higher and bilingual ability preferred to

Qualifications

• MSW from an accredited university.
• Minimum 2 years in case management, social advocacy, and/or mental health with geriatric population.
• BLS certification or higher per AHA/Red Cross guidelines.
• Bilingual Spanish/English or other language strongly preferred.

Responsibilities

• Provide psychosocial evaluation and care planning for PACE participants.
• Coordinate care, case management, and disenrollment processes.
• Engage in individual and group interventions; educate participants and families.
• Collaborate with the Interdisciplinary Team to optimize health outcomes.
