Job summary

AltaMed is seeking a dedicated Social Work/Patient Services professional to support the PACE IDT and coordinate Medi-Cal/Medicare workflows. You will handle inquiries, complaints, appeals, and disenrollments, and assist with linkage to community resources while collaborating with MSWs and interdisciplinary teams to meet program goals.

The role emphasizes high-quality customer service, eligibility knowledge, and on-site participant experience activities, with bilingual skills strongly preferred.

Qualifications

• Bachelor's degree in social work or another health or human services field is required.
• Minimum 2 years of customer service or case management experience, preferably in healthcare.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits incl. applications/renewals.
• BLS/First Aid certification is required.
• Experience in PACE or healthcare settings serving frail/elderly preferred.
• Bilingual Spanish/English, Chinese/English or another second language strongly preferred.

Responsibilities

• Coordinate PACE services and promote participant satisfaction and retention.
• Address participant/family service questions, requests, complaints, and appeals.
• Process involuntary disenrollments and support on-site satisfaction surveys.
