Job summary

AltaMed Health Services Corp. is seeking a Social Work Assistant to coordinate PACE services within the IDT and maintain high participant satisfaction through strong customer service skills.

You will handle inquiries, complaints, and appeals, processing disenrollments and on-site surveys, while linking participants to community resources and performing case management as needed. Collaboration with MSWs and teams is essential.

Qualifications

• Bachelor's degree in social work or another health or human services field required.
• Minimum of 2 years of customer service or case management experience.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits including applications/renewals.
• BLS/First Aid certificate required.
• PACE or healthcare experience with frail/elderly is preferred.
• Bilingual abilities (Spanish/English, Chinese/English, or another language) preferred.

Responsibilities

• Coordinate PACE services within the IDT and promote participant satisfaction and retention.
• Address participant and family service questions, requests, complaints, appeals, and incident reporting.
• Process involuntary disenrollments and conduct on-site satisfaction surveys.
• Linkparticipants to community resources and perform case management.
• Collaborate with MSWs and interdisciplinary teams to meet program goals while following regulations.
