Job summary

AltaMed Health Services Corp. seeks a social work professional to coordinate PACE services and enhance participant satisfaction through high-quality service.

You will address participant questions, complaints, appeals, and incident reporting, ensuring Medi-Cal and Medicare issues are resolved via established workflows. Responsibilities include processing involuntary disenrollments, conducting on-site satisfaction surveys, and linking families to community resources while collaborating with MSWs

Qualifications

• Bachelor’s degree in social work or a health/human services field required.
• Minimum 2 years of customer service or case management experience.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits required.
• Bilingual abilities strongly preferred.

Responsibilities

• Assist PACE IDT in coordinating essential PACE services.
• Support participant and family questions, requests, complaints, and appeals.
• Address Medi-Cal/Medicare issues via established workflows.
• Process involuntary disenrollments and conduct on-site satisfaction surveys.
• Link participants to community resources and perform case management.
