Job summary

AltaMed Health Services Corp. is seeking a Senior Care Advocate II to coordinate PACE services and support participant satisfaction. You will handle questions, requests, complaints, and incident reporting, ensuring Medicare and Medi-Cal workflows are followed for timely resolutions.

Qualifications include a Bachelor’s in social work or related field, 2+ years in customer service or case management, and knowledge of Medi-Cal/Medicare. Bilingual skills are strongly preferred.

Qualifications

• Minimum 2 years of customer service or case management experience.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits.
• BLS/First Aid certification required.
• Experience in PACE or healthcare settings serving frail or elderly preferred.
• Bilingual abilities strongly preferred.

Responsibilities

• Coordinate essential PACE services with the IDT and promote participant satisfaction and retention.
• Address participant and family service questions, requests, complaints, appeals, and incident reporting.
• Manage Medi-Cal and Medicare issues via established workflows for prompt resolution.
• Process involuntary disenrollments and conduct on-site satisfaction surveys.
• Link participants and families to community resources and perform case management tasks as needed.
• Collaborate with MSWs and interdisciplinary teams to meet goals within regulations.
