AltaMed Health Services Corp. is seeking a Senior Care Advocate II in Lynwood to support the PACE IDT, coordinating services and improving participant satisfaction and retention through high-quality customer service.

You will handle inquiries, complaints, and appeals, ensure proper Medi-Cal and Medicare workflows, and assist with disenrollments and on-site satisfaction surveys. Join a team delivering compliant, compassionate care to seniors.

Qualifications

• Bachelor's degree in social work or related health/human services field is required.
• Minimum 2 years of customer service or case management experience, healthcare preferred.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits; including applications and renewals.
• BLS/First Aid certification required.
• Experience in PACE or healthcare settings serving frail/elderly preferred.
• Bilingual: Spanish/English, Chinese/English, or another second language preferred.

Responsibilities

• Coordinate essential PACE services with the IDT and promote participant satisfaction.
• Address participant/family questions, requests, complaints, appeals, and incident reporting.
• Ensure Medi-Cal and Medicare issues are resolved via established workflows.
• Assist with involuntary disenrollments, satisfaction surveys, and center-wide participant experience activities.
• Link participants to community resources and perform case management as needed.
• Collaborate with MSWs and interdisciplinary teams to meet program goals while following regulations.
