Job summary

AltaMed is seeking a dedicated professional to join their social work department, assisting in the coordination of essential PACE services while enhancing participant satisfaction. You will support participant and family inquiries, manage service requests, and ensure effective resolution of Medi-Cal and Medicare issues.

The ideal candidate holds a Bachelor’s degree in social work, possesses at least 2 years in customer service or case management, and has strong knowledge of relevant healthcare

Qualifications

• Bachelor's degree in social work or health services required.
• Minimum of 2 years in customer service or case management.
• Knowledge of Medi-Cal, Medicare, and Social Security benefits required.
• BLS/First Aid certificate is required.
• Preferred experience in PACE or healthcare settings for frail or elderly.
• Bilingual in Spanish/English, Chinese/English, or another language preferred.

Responsibilities

• Assist PACE IDT in coordinating essential services.
• Support and address participant and family service questions.
• Ensure Medi-Cal and Medicare issues are effectively addressed.
• Process involuntary disenrollments.
• Facilitate on-site satisfaction surveys and engage in activities.
