TRU Community Care in Lafayette is seeking a PACE Care Coordinator to schedule appointments with specialists and manage CMS compliance for participant care. You will coordinate with the medical team and outside providers, maintaining EMR authorizations and accurate documentation.

The role requires 3+ years in medical administration, proficiency with Word, Excel and PowerPoint, and strong team collaboration.

Qualifications

• 3+ years administrative experience in medical related field.
• Must be computer proficient and able to work with various databases, and be competent with Word, Excel and PowerPoint programs.
• Ability to collaborate with team and manage several projects simultaneously.
• Experience working with the frail or elderly population preferred.
• Experience working with individuals with mental health needs desirable.

Responsabilités

• Assist in prioritizing the scheduling of participants.
• Schedule all appointments for participants with specialists and outside medical appointments when requested by the medical team. Will create authorizations for scheduled appointments within the EMR.
• Documents in the medical record as appropriate to scope of practice.
• Perform any necessary filing or clerical duties necessary to complete charting requirements.
