Job summary

One Senior Care, LLC. is seeking a Medical Assistant - PACE Clinic Coordinator to organize and prepare provider-facing medical records and documentation for Participant visits.

You will work with CDI Nurses, Medical Records, and clinical teams to ensure accurate, complete, and timely information. The role emphasizes medical coding expertise, EMR familiarity, and strong organizational skills to support quality care for PACE Participants.

Qualifications

• Experience with medical coding with a CC focus.
• Strong attention to detail and organization in handling clinical information.
• Familiarity with electronic medical records and healthcare documentation practices.

Responsibilities

• Gather and organize existing medical records and approved clinical documentation for provider review.
• Prepare provider-facing documentation packets and templates before scheduled Participant visits.
• Review work queues and retrieve approved reports and supporting documentation.
• Track documentation status and route items to the appropriate provider or CDI Nurse.
• Maintain Participant confidentiality and HIPAA compliance.
