myPlace Health in Downtown Los Angeles (PACE program) seeks a Scheduler to coordinate participant appointments across medical, dental, therapy, and specialty services. You will align care plans with the interdisciplinary team, arrange transportation, and support onboarding for new participants, ensuring regulatory timing and smooth operations.

This role emphasizes clear communication, problem-solving, and precise scheduling in EMR systems to maintain accurate records and timely care for seniors

Qualificações

• Education: A High School Diploma or GED is required.
• Experience: At least 2 years in medical scheduling or healthcare administration.
• Elderly care: At least 1 year working with frail elderly participants.

Responsabilidades

• Coordinate and schedule participant appointments across medical, dental, therapy, and specialty services.
• Facilitate external appointments and referrals with referrals, authorizations, and transportation.
• Organize orientation and initial assessments for new participants across multiple disciplines.
• Optimize scheduling across care settings and minimize conflicts.
• Collaborate with Transportation, Home Care, Medical Records, and Clinic teams.
• Act as liaison between Senior Center Manager, Clinic Staff, Medical Records, Home Care, and Transportation Coordinators.
• Place confirmation calls and manage follow-ups for external provider appointments.
• Manage scheduling conflicts and urgent requests to ensure continuity of care.
• Maintain accurate documentation in EMR or scheduling software.
• Other duties as assigned.
