Job summary

PACE Your LIFE seeks a Medical Scheduler to coordinate referrals to outside specialists and ensure timely access to care for participants. The role emphasizes clear communication among providers, participants, and external offices, while supporting continuity of care and confidentiality.

The position involves updating EMR systems, tracking referrals, and collaborating with care teams to optimize scheduling efficiency and participant outcomes.

Qualifications

• Must read, write and speak English (clear communication).
• Outstanding organizational and communication skills are required.
• Experience working with frail or elderly populations preferred.
• Three years of scheduling experience preferred.
• Proficiency with Outlook, Word, and Excel strongly preferred.

Responsibilities

• Coordinate appointments with outside specialists (cardiology, neurology, endocrinology, etc.).
• Confirm availability with external offices and align with participant preferences.
• Communicate appointment needs between PACE Your LIFE clinical teams and external offices to maximize services.
• Track referral status and follow up to ensure appointments are completed.
• Notify participants and caregivers of scheduled appointments and provide instructions.
• Answer questions regarding referrals and visits; cancel and reschedule as needed.
• Maintain accuracy of referral documentation and records in EMR.
• Generate reports on scheduling completion rates and participant follow-up compliance.
