Job summary

One Senior Care is seeking a Medical Assistant - PACE Clinic Coordinator to organize medical records and prepare provider-facing documentation for PACE participants. You will gather records, build documentation support materials, manage work queues, and ensure clinicians have complete information before visits.

The role emphasizes strong CC-focused medical coding, experience with electronic medical records, and collaboration with CDI Nurses, Medical Records, and clinical teams to support

Qualifications

• High School Diploma or GED required; Medical Assistant training preferred.
• Experience with medical coding and CC-focused documentation.
• Proficiency with electronic medical records and provider documentation.

Responsibilities

• Gather and organize existing medical records for provider review.
• Prepare provider-facing documentation packets before visits.
• Review work queues and retrieve reports and supporting documentation.
• Organize hospital, ED, SNF, labs, medications, vitals and care plans.
• Maintain timely, accurate, neutral documentation and confidentiality.
