Job summary

NEMS is seeking a PACE Navigator in Daly City to coordinate participant appointments, referrals, transportation, and services, ensuring timely access to healthcare under PACE program requirements.

The role serves as a primary contact for participants, caregivers, providers, and community partners, delivering excellent customer service and maintaining accurate documentation in Epic. Strong English and Chinese language skills are required.

Qualifications

• High School Diploma or equivalent.
• Minimum two years of experience in healthcare scheduling, patient coordination, medical office administration, or customer service.
• Strong organizational, communication, and time management skills.
• Ability to prioritize multiple tasks in a fast-paced environment.
• Proficiency with Microsoft Office applications.
• Ability to learn and effectively utilize Epic or other electronic health record systems.
• Excellent customer service and interpersonal skills.
• Attention to detail and ability to work independently and collaboratively.

Responsibilities

• Schedules participant appointments according to program requirements and timeframes.
• Coordinates appointments with specialists, hospitals, imaging centers, laboratories, and external providers.
• Schedules internal providers and interdisciplinary team members.
• Coordinates transportation, escorts, meal services, center attendance, and other participant needs.
• Monitors referrals and follows up to ensure scheduling and completion of appointments.
• Contacts participants and caregivers regarding reminders, schedule changes, and follow-ups.
• Documents interactions and scheduling activities in the electronic health record (Epic).
• Maintains appointment schedules and current demographic and insurance information.
• Communicates with providers and care teams to ensure coordinated care.
• Resolves scheduling conflicts and escalates issues as needed.
• Generates reports and supports daily operations and quality improvement.
• Participates in meetings and workflow improvements.
• Maintains confidentiality and complies with HIPAA and policies.
• Performs other duties as assigned.
