Job summary

Trinity Health PACE is seeking a Part-time Scheduling Coordinator to manage participant appointments and coordinate across the interdisciplinary team. You will work closely with transportation, participants, caregivers, and center staff to keep calendars accurate and up to date.

The role requires strong communication, organization, and customer service skills, plus experience with scheduling systems and an Elderly/population focus. A flexible, patient-centered approach is essential.

Qualifications

• High School Diploma or equivalent required.
• Associate degree preferred.
• 1 year scheduling experience required or strongly preferred.
• 1 year experience with frail/elderly population or retraining on hire.
• 1 year customer service experience preferred.
• Strong written and verbal communication skills, flexibility to adapt to change.
• Proficiency with MS Word, Excel, PowerPoint; basic email/calendaring skills.

Responsibilities

• Coordinate and schedule external appointments per care plans.
• Notify managers of scheduling changes requiring physician orders.
• Record scheduling updates daily; respond promptly to referrals.
• Coordinate with departments for participant appointments.
• Ensure nursing completes participant education before appointments.
• Create EPIC authorizations and transportation appointments.
• Remind participants/caregivers of appointments.
