Job summary

Polaris Pharmacy Services is seeking a Staff Pharmacist to manage daily dispensing operations for our long-term/post-acute care pharmacy in West Mifflin, PA. You will ensure accuracy and quality in medication labeling while adhering to state and federal guidelines.

You will also oversee sterile IV preparations, maintain patient records, and provide clinical support to staff and patients. The position requires flexibility for evening, weekend, and holiday coverage and a commitment to regulatory

Qualifications

• Pharmacist must hold an active PA license with good standing.
• Experience in LTC/acute-care settings is preferred but not required.
• Bachelor’s degree or Doctorate of Pharmacy from an accredited school.
• Management experience is a plus for this role.

Responsibilities

• Verify accuracy of all new and refill prescription labels and orders.
• Maintain resident medication history and related clinical data.
• Assist in controlled substances handling and perpetual inventory.
• Collaborate with technicians for reconciliation of inventories.
• Provide clinical answers to drug-related questions from staff and patients.
• Document communications with physicians and healthcare team in the medical record.
• Prepare and supervise sterile IV products when required.
• Provide evening, weekend, and holiday coverage as scheduled.
• Comply with HIPAA and all state/federal regulations.
• Contribute to a safe, efficient workflow and meet business needs.
