Job summary

Polaris Pharmacy Services is seeking a Staff Pharmacist to handle day-to-day dispensing duties for long-term and post-acute care. You will ensure label accuracy, maintain patient medication histories, and manage controlled substances in a compliant setting.

The role requires an active pharmacist license, a PharmD/Bachelor's degree, and experience in acute care or production pharmacy, with IV admixture knowledge preferred. Flexible scheduling including evenings and weekends is expected.

Qualifications

• Active Pharmacist license in good standing.
• Bachelor’s degree or PharmD from an accredited school.
• Minimum of two (2) years’ experience in an acute-care setting or one (1) year as a production pharmacist preferred.
• Experience and knowledge base of intravenous admixtures services and intravenous therapy desired.
• Strong communication and teamwork skills; ability to problem solve.

Responsibilities

• Verify completeness and accuracy of all new and refill labels and orders.
• Maintain resident database with current medical history and clinical data.
• Assist in ordering, handling, receiving, documenting, and storing controlled substances; maintain perpetual inventory.
• Provide clinical expertise by responding to drug-related questions from staff and patients.
• Prepare and supervise sterile IV drug products or solutions.
• Provide evening, weekend, and holiday coverage as required.
• Follow HIPAA and other government regulations.
