Polaris Pharmacy Services is seeking a Staff Pharmacist to manage day-to-day dispensing for long-term/post-acute care and ensure compliance with quality standards and regulations. The role requires accurate labeling, data maintenance, and collaboration with care teams.

You will supervise sterile IV products, handle controlled substances, and provide clinical answers while rotating evenings, weekends, and holidays. Licensure in good standing and PharmD are essential.

Qualifications

• Active Pharmacist license in good standing in the state of practice.
• Duties may include clinical tasks, dispensing, and supervision.
• Experience in LTC or acute care settings preferred.

Responsibilities

• Verify accuracy of new and refill labels and orders for accuracy.
• Maintain resident medical history, labs, and concurrent medications.
• Assist in handling controlled substances and perpetual inventory.
• Provide clinical expertise by answering drug-related questions.
• Document communications with physicians and care team in records.
• Prepare and supervise sterile IV products.
• Provide coverage evenings, weekends, and holidays as needed.
• Follow HIPAA and government regulations.
