Job summary

Polaris Pharmacy Services of Pennsylvania LLC is seeking a Staff Pharmacist to support day-to-day dispensing for long-term/post-acute care and related specialties. The role requires adherence to quality standards and federal/state guidelines, with an overnight 7 on/7 off schedule.

Responsibilities include verifying labels, maintaining resident med histories, handling controlled substances, and preparing IV products while providing clinical guidance to staff and patients.

Qualifications

• Pharmacist licensure in good standing in the state of practice.
• Bachelor’s degree or PharmD from an accredited school.
• Experience with IV admixtures is desirable.

Responsibilities

• Verify completeness and accuracy of labels and orders.
• Maintain resident med history and clinical data in the database.
• Assist in ordering, handling, and storage of controlled substances.
• Provide clinical drug information to staff and patients.
• Prepare sterile IV drug products and solutions.
• Support evening, weekend, and holiday coverage.
