Polaris Pharmacy Services is seeking a Staff Pharmacist to handle day-to-day dispensing in a long-term/post-acute care setting. You will ensure accuracy, adhere to quality standards, and follow state and federal guidelines.

Qualifications

• Current Pharmacist license active in the state of practice.
• Bachelor's degree or PharmD from an accredited school.
• Minimum of two years recent experience in an acute-care setting or one year as a production pharmacist.
• Experience and knowledge base of intravenous admixtures and therapy desired.

Responsibilities

• Verify accuracy of labels and orders for dispensing.
• Maintain resident medical history and data.
• Manage controlled substances inventory and schedules.
• Reconcile Controlled Substances perpetual inventory as required by management.
• Provide clinical drug information to staff and patients.
• Document communications in medical records.
• Prepare and supervise sterile IV products or solutions.
• Provide coverage for evenings, weekends, and holidays.
• Comply with HIPAA and government regulations.
• Other duties as assigned.
