Job summary

Polaris Pharmacy Services of Ohio LLC is hiring a Staff Pharmacist to manage dispensing duties in a long-term/post-acute care setting in Ohio. You will ensure accuracy in labeling, maintain drug inventories, and support clinical decisions while adhering to HIPAA and regulatory standards.

This role requires an active Ohio Pharmacist license and relevant acute-care or production pharmacy experience. It includes evening/weekend coverage and benefits as part of a full-time team in a 24-hour

Qualifications

• Current Pharmacist license active in the state of practice.
• Bachelor’s degree or Pharmacy Doctorate from an accredited School of Pharmacy.
• Minimum of two (2) years recent experience in an acute-care setting or one (1) year as a production pharmacist.

Responsibilities

• Verify completeness and accuracy of all new and refill labels and orders.
• Maintain resident database with current and past medical history and medications.
• Assist in ordering, handling, receiving, documenting, and storing controlled substances.
• Provide clinical drug information to staff and patients and document communications.
• Prepare and supervise sterile IV drug products or solutions.
• Provide evening, weekend, and holiday coverage as required.
• Follow HIPAA and state/federal regulations.
