Job summary

Polaris Pharmacy Services of Northern CA - Livermore, CA is seeking a Staff Pharmacist to manage day-to-day dispensing operations for a long-term and post-acute care pharmacy. You will ensure quality and compliance with state and federal guidelines while maintaining patient-focused service.

Responsibilities include verifying labels, maintaining resident records, handling controlled substance inventory, and providing clinical guidance to staff and patients.

Qualifications

• Current Pharmacist license active and in good standing in the state of practice.
• Minimum of two (2) year’s recent experience in an acute-care setting or one (1) year experience as a production pharmacist preferred.

Responsibilities

• Verify completeness and accuracy of new and refill labels and orders.
• Maintain resident database with current medical history and medications.
• Assist in handling controlled substances and maintain inventory for schedule II drugs.
• Reconcile Controlled Substances inventory per state and federal regulations.
• Provide clinical expertise by addressing drug-related questions from staff and customers.
• Document communications with physicians, nurses, and patients in medical records.
• Prepare and supervise sterile IV drug products.
• Provide evening, weekend, and holiday coverage as required.
• Follow HIPAA and other government regulations.
