Job summary

Polaris Pharmacy Services is hiring a Staff Pharmacist to support day-to-day dispensing for long-term and post-acute care. You will ensure accuracy of labels, maintain patient medical histories, and manage controlled substances in coordination with the pharmacy team.

The role requires a current pharmacist license, a PharmD or PharmB, and clinical collaboration with physicians, nurses, and residents to ensure safe therapy and regulatory compliance.

Qualifications

• Active Pharmacist license in good standing in the practicing state.
• Pharmacy degree (PharmD or PharmB) from an accredited school.
• Experience in acute-care settings is preferred.

Responsibilities

• Verify accuracy of new and refill labels and orders.
• Maintain resident database with medical history and medications.
• Assist with ordering, handling, and storage of controlled substances.
• Provide clinical guidance by answering drug-related questions.
• Supervise sterile IV drug products preparation and ensure compliance.
