You will assess hopes and desires, help individuals participate in community activities, advocate for rights, provide emotional and daily living support, and document progress according to policy.

Qualifications

• High School Diploma required (as stated).
• Driver's license (USA) required.
• Experience working with individuals with mental health diagnoses or developmental disabilities or substance use disorders (preferred/varies by population).

Responsibilities

• Assess each person’s hopes, dreams, and desires and provide supports to facilitate achievement.
• Assist people in participating fully in their community consistent with their interests.
• Ensure rights of individuals are protected and promote advocacy and self-determination.
