Job summary

Qualifications

• Experience working with people who have mental health diagnoses or developmental disabilities; not required but preferred.
• Must have a valid US driver’s license.

Responsibilities

• Assess individuals served for hopes and goals and provide supports to achieve them.
• Help individuals develop community relationships and natural supports.
• Assist with participating in community activities and daily living tasks.
• Protect and promote the rights of individuals; assist with advocacy.
• Document progress, goals, incidents, and timesheets.
• Provide transportation and arrange travel as needed.
• Support safety procedures and emergency readiness.
