Job summary

Creative Pathways Inc. seeks an SUD Peer Support Specialist / Case Manager to deliver outpatient substance abuse treatment services. You will counsel individuals with alcohol and other drug addictions, teach coping skills, and coordinate care with treatment teams.

Responsibilities include creating treatment plans, assessing functional status, maintaining records, and providing aftercare planning. Certification and a DC CAC level I/II authorization are required, with clinical supervision

Qualifications

• High school diploma with experience working with the substance abuse population.
• A graduate or postgraduate degree in sociology, social work, arts, science or related field.
• Certification in alcohol and drug counseling is required.
• A master's degree in social work, arts or science is preferred for counseling centers and hospitals.
• Licensure as a substance abuse counselor is mandatory.

Responsibilities

• Develop comprehensive individualized treatment plans and educate clients.
• Assess client and family systems and daily living functioning.
• Maintain case records and provide required documentation.
• Provide follow-up and aftercare planning; discharge inactive clients as needed.
• Provide emergency services during and after work hours as assigned.
• Attend staff meetings and clinical supervision.
