Job summary

Advocate Health is seeking a dedicated professional to manage addiction casework and supervise students within Substance Use Service. You will conduct comprehensive biopsychosocial assessments, develop diagnostic summaries, and create individualized treatment plans while coordinating with patients, families, and the care team.

The role requires experience with 12-step recovery concepts, group counseling, and clinical supervision to support ongoing patient recovery and program development.

Qualifications

• Associates Degree in Behavioral Health or equivalent; Bachelors preferred.
• 2 years of experience in the field.
• Ability to recognize the needs and behaviors of diverse age groups.

Responsibilities

• Responsible for case management of addiction patients and families.
• Complete biopsychosocial assessments.
• Formulate diagnostic summaries and treatment plans.
• Coordinate input from team, family and patient goals.
• Revise treatment plans as progress dictates.
