Top Echelon Software, LLC is seeking a Physical Therapist Assistant (Outpatient) to provide skilled therapy under the supervision of a licensed Physical Therapist in Montour Falls, NY. The role supports individualized treatment plans, patient education, and discharge planning within a dynamic outpatient setting.

You will assist with therapeutic exercises, mobility activities, and use of assistive devices, while ensuring compliance with state regulations and clinic policies.

Qualifications

• Education: Associate's degree in an accredited Physical Therapist Assistant program.
• Licensure: Current New York State PTA license or eligibility for licensure.
• Certification: Current CPR Certification.
• Training: Annual mandatory safety in-services and HealthStream e-Learning.

Responsabilités

• Provides skilled physical therapy treatment under the supervision of a licensed Physical Therapist.
• Assists with implementing treatment plans.
• Instructs and assists patients with therapeutic exercises and activities.
• Assists with safe use of assistive devices.
• Administers modalities per PT guidance.
• Monitors and documents patient responses; reports progress to PT.
• Communicates with PT on progress and modifications.
• Participates in discharge planning and home exercise instructions.
• Educates patients and families on exercises, safety and home program.
• Documents care and charges in EMR.
• Maintains compliance with NYS requirements and facility policies.
