Job summary

Carle Health is seeking a Physical Therapist to plan and administer treatment plans designed to alleviate pain, restore function, prevent disabilities, and promote overall fitness and health of patients. Serves as a role model in delivery of professional services and as a clinical resource for staff and students.

The role requires licensure as a PT (IDFPR) and BLS within 30 days, a Bachelor's in Physical Therapy, and outpatient orthopedic experience.

Qualifications

• Licensed Physical Therapist (PT) with IDFPR certification.
• Basic Life Support (BLS) within 30 days - American Heart Association (AHA).
• Bachelor's Degree: Physical Therapy (Required).

Responsibilities

• Performs patient transfers with assistance as needed and uses safe body mechanics.
• Designs an appropriate plan of care with goals, treatment, outcomes, and discharge planning.
• Adjusts care based on patient response and evidence.
• Educates patients, families, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Performs examinations using reliable methods relevant to chief complaint, screening, and history.
• Screens patients to determine need for therapy services.
• Monitors care delivered by support personnel and provides feedback.
• Communicates with the medical team.
• Documents care per department standards and practice guidelines.
• Meets customer service and productivity standards.
• Serves as clinical instructor when assigned by team leader or supervisor.
• Participates in team and department activities and meetings.
• Performs reporting and other duties as assigned.
