Qualifications

Responsibilities

• Performs patient transfers with assistance when needed.
• Demonstrates safe body mechanics and compassionate care.
• Designs and implements a plan of care with goals and discharge plan.
• Adjusts treatment based on patient response and evidence.
• Educates patients and families using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examination using reliable methods relevant to chief complaint.
• Screens patients to determine need for therapy services according to standards.
• Monitors care provided by support staff and provides feedback.
• Communicates with medical team and documents per standards.
