Job summary

Carle Health is seeking a licensed Physical Therapist to plan and administer PT treatments aimed at relieving pain, restoring function, and promoting overall health. The role acts as a clinical resource for staff and students and requires strong patient education and team collaboration.

Qualifications

• Licensed Physical Therapist (PT) per IDFPR; BLS within 30 days per AHA.

Responsibilities

• Demonstrates patient transfers with assistance as needed.
• Adheres to safe body mechanics to protect staff and patients.
• Designs and carries out an individualized plan of care with goals and discharge plan.
• Adjusts care based on patient response and available evidence.
• Educates patients, families, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examinations using reliable methods relevant to chief complaint, screening, and history.
• Screens patients to determine need for therapy services and regulatory compliance.
• Monitors care delivered by support personnel and provides feedback.
• Communicates with the medical team and documents per standards.
• Meets customer service and productivity standards; serves as clinical instructor when assigned.
• Participates in team, program, and department activities and meetings.
