Job summary

Carle Health is seeking a Licensed Physical Therapist to plan and administer treatment programs designed to alleviate pain, restore function, and promote overall fitness and health for patients. The role also acts as a clinical resource and mentor for staff and students.

Required qualifications include a Bachelor's Degree in Physical Therapy, active PT license (IDFPR) and Basic Life Support (BLS) within 30 days. Outpatient orthopedic experience is preferred.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor
• Drives safe body mechanics and compassionate patient care with dignity and respect
• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan
• Adjusts care based on patient response and evidence
• Educates patients, family, caregivers, students and other health care providers
• Evaluates clinical findings to determine PT diagnoses and outcomes
• Performs examination with reliable methods relevant to chief complaint
• Screens patients to determine need for therapy services
• Monitors care delivered by support personnel and provides feedback
• Communicates with medical team
• Performs documentation
• Meets customer service and productivity standards
• Serves as clinical instructor when assigned
• Participates in team and department meetings
• Performs reporting and other duties as assigned
