Meritas Advisory Group, LLC is seeking an experienced Outpatient Physical Therapist for a full-time role in Cayuga, NY. You will provide hands-on rehabilitation, develop individualized plans, and collaborate with an established outpatient team to support diverse patient needs.

Qualifications

• Experience in outpatient physical therapy settings preferred.
• Must hold or be eligible for a valid Physical Therapist license in NY.
• BLS certification required within 90 days of hire.

Responsibilities

• Conducting initial evaluations and developing individualized treatment plans for each patient.
• Administering therapeutic treatments (exercises, manual therapy, modalities) and patient education.
• Educating patients on exercise programs, posture correction, and injury prevention for long-term recovery.
• Collaborating with multidisciplinary care team to provide comprehensive patient-centered care.
• Monitoring progress and adjusting treatment plans to meet rehabilitation goals.
• Documenting plans, progress notes and discharge summaries per policies and standards.
• Providing excellent patient education to empower patients in managing their rehab.
• Maintaining a clean, safe, and organized environment following infection control guidelines.
