Job summary

Talent With Heart in the Finger Lakes region seeks an Outpatient Physical Therapist to join an established rehabilitation team in Cayuga, NY. This full-time role emphasizes hands-on care for a diverse patient mix and collaborative, patient-centered outcomes.

You will evaluate, treat, educate, and work with a multidisciplinary team to support recovery and functional improvement. A DPT is required with NY licensure eligibility and BLS within 90 days of hire.

Qualifications

• Must hold or be eligible to obtain a valid Physical Therapist license in New York.
• Doctor of Physical Therapy (DPT) or equivalent from an accredited program is required.
• Basic Life Support (BLS) certification required within 90 days of hire.

Responsibilities

• Conduct initial evaluations and develop individualized treatment plans.
• Administer therapies including exercises, manual therapy, modalities, and patient education.
• Educate patients on exercise programs, posture, and injury prevention.
• Collaborate with multidisciplinary care team to provide comprehensive care.
• Monitor progress and adjust plans to meet rehabilitation goals.
• Document treatment plans, progress notes, and discharge summaries per policy.
• Provide excellent patient education to empower self-management.
• Maintain a clean, safe clinical environment according to infection control guidelines.
