Job summary

Mercy Home Health is seeking a licensed Physical Therapist to perform initial and ongoing assessments and provide skilled interventions across inpatients, outpatients, pediatrics, and off-site locations.

You will develop and adjust treatment plans, document progress in the EMR, and supervise therapy assistants while upholding safety and professional standards. BLS is preferred or required within 30 days in some facilities.

Qualifications

• Knowledge of age-specific standards and guidelines for pediatric, adult, and geriatric patients.
• Proficient in basic data entry, word processing, email, and records management.
• Ability to analyze patient condition, progress, and adjust plans accordingly.

Responsibilities

• Evaluates patients and develops treatment plans with specific goals.
• Provides daily direct therapy following the plan of care and standards of practice.
• Maintains accurate treatment records and progress notes in the EMR; coordinates care.
• Maintains productivity while ensuring quality care and compliance with policies.
• Supervises therapy assistants and supports professional growth.
• Supports safety, infection control, and annual compliance requirements.
