Fresenius Medical Care is seeking an Outpatient RN CAP2 to provide and coordinate clinically competent care for an assigned patient group undergoing hemodialysis. The role includes assessment, planning, intervention and evaluation with collaboration across RNs, LVN/LPNs and Patient Care Technicians.

Qualified candidates will have a graduate nursing degree, current state licensure, and CPR/BLS certification.

Formación

• RN CAP2 entry level designation into the CAP for new employees meeting criteria.
• Minimum 2 years of nephrology nursing experience.
• Current state nursing license and CPR/BLS certification required.

Responsabilidades

• Perform ongoing collection and analysis of patient data before, during and after hemodialysis.
• Document in the patient record and adjust treatment plan as needed.
• Provide patient education and collaborate with the care team.
• Administer medications per prescriptions or approved algorithms and monitor effectiveness.
• Lead, coach and mentor team members and support preceptor roles as needed.
