Job summary

Fresenius Medical Care in the United States is seeking an Outpatient RN CAP 2 to provide and coordinate clinically competent care for an assigned group of patients undergoing hemodialysis. The role includes assessment, planning, intervention, and evaluation, with potential delegation to RNs, LVN/LPNs, and Patient Care Technicians as needed.

The RN CAP 2 will participate in teaching, leadership functions, and quality improvement activities within the ESKD care team, under supervision and guidance

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful CPR BLS Certification.
• Entry level RN CAP 2 with minimum of 2 years of nephrology nursing experience.

Responsibilities

• Perform ongoing collection and analysis of patient data pre-, during-, and post-hemodialysis and adjust the care plan accordingly.
• Assess, educate, and document patient/family learning needs related to dialysis care.
• Direct and provide safe and effective delivery of hemodialysis therapy with direct and ancillary staff.
• Administer medications as prescribed or per approved algorithms.
• Initiate or assist with emergency response measures.
• Serve as a resource, mentor, and role model for new employees.
• Ensure correct laboratory collection, processing and shipping procedures.
• Identify expected outcomes and update nursing assessment and plan of care with the interdisciplinary team.
• Participate in education and quality improvement projects; may serve as Preceptor to new employees.
• Complete CAP requirements to maintain or advance; perform other duties as assigned.
