Job summary

Fresenius Medical Care seeks an RN CAP 2 to support the dialysis patient team with assessment, planning, intervention and evaluation. The role includes coordinating care, educating patients and families, and guiding staff under supervision.

You will participate in quality improvement and may serve as a mentor for new employees. Requires RN licensure, CPR/BLS certification, and nephrology experience.

Qualifications

• Entry level RN CAP 2 designation into CAP for new employees meeting criteria
• Provision and coordination of clinically competent care including assessment, planning, intervention and evaluation
• May delegate tasks to direct care staff and collaborate with the ESKD health care team

Responsibilities

• Collect and analyze patient data during hemodialysis and document in the medical record
• Adjust treatment plan as indicated and notify supervisor as needed
• Educate patients and families about transplant and treatment options
• Direct and provide safe delivery of hemodialysis therapy and supervise care staff
• Participate in education and quality improvement projects at facility/area level
