Job summary

Fresenius Medical Care is seeking an entry-level Registered Nurse for the Outpatient CAP program. The role focuses on assessment, planning, intervention, and evaluation for an assigned patient group undergoing hemodialysis, with delegation duties to PCTs/LPNs/LVNs as needed.

As part of the ESKD care team, you will participate in decision-making, education, leadership, and quality improvement activities that enhance patient outcomes and facility operations.

Qualifications

• Graduate of an accredited School of Nursing
• Current active state nursing licensure
• CPR/BLS certification
• Minimum 9 months RN experience
• Minimum 3 months dialysis experience

Responsibilities

• Performs systematic data collection and analysis during dialysis and documents in the medical record.
• Provides patient education to patients and families.
• Directs safe delivery of hemodialysis therapy and collaborates with care staff.
• Administers medications per prescribed protocol.
• Participates in staff training and orientation.
• Identifies expected outcomes and updates care plans with Interdisciplinary Team
