Job summary

Fresenius Medical Care in Palmdale, CA seeks a professional RN CAP 2 for Outpatient CAP career advancement. You will coordinate and deliver hemodialysis care, collaborate with the care team, and educate patients and families. This entry-level CAP2 may advance from RN CAP1 or newly qualified for CAP.

Responsibilities include data collection during dialysis, adjusting care plans, and mentoring staff. A CRM focus, leadership, and quality improvement participation are expected.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current or successful completion of CPR BLS Certification.

Responsibilities

• Performs all essential functions under supervision and with guidance.
• Assesses, analyzes and documents patient data during dialysis and adjusts treatment plan.
• Educates patients and families on basic learning needs and education.
• Directs daily provision of safe and effective hemodialysis therapy with staff.
• Administers medications as prescribed or by approved algorithms.
• May serve as a Preceptor to new employees.
