Job summary

Fresenius Medical Care is seeking an entry-level RN to join the Outpatient CAP1 program. You will assess, plan, intervene, and evaluate care for an assigned patient group and coordinate with RNs, LPNs/LVNs, and Patient Care Technicians in a dialysis setting.

As a member of the ESKD health care team, you will participate in teaching, leadership, and quality improvement activities, with ongoing CAP requirements to advance within the program.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• CPR/BLS Certification.
• Must meet practice requirements in the state employed.
• Entry level for RNs with less than 2 years of nephrology nursing experience.
• Chronic/acute hemodialysis experience preferred.

Responsibilities

• Performs all essential functions under the direction of the Supervisor with guidance from the Clinical Educator, Preceptor or colleagues.
• Collects and analyzes patient data during dialysis and updates the care plan as needed.
• Educates patients and families about care plans and treatment options.
• Delivers hemodialysis therapy safely in collaboration with other staff.
• Administers medications as prescribed and documents outcomes.
• Participates in emergency response measures when needed.
• Supports staff training and orientation for new personnel.
• Ensures proper specimen collection procedures are followed and labs are processed correctly.
• Documents nursing assessments and plans of care with the interdisciplinary team.
• Works with transplant and modality options discussions with patients.
