Job summary

Fresenius Medical Care in Wisconsin is seeking an Outpatient RN CAP 2 to join the End Stage Kidney Disease care team. The role supports clinical assessment, planning, intervention, and evaluation for assigned patients and may involve supervising other staff.

Responsible for education, care coordination, and quality improvement activities in the facility, with emphasis on safe, effective hemodialysis delivery and adherence to policy and regulatory standards.

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state RN licensure.
• Current or successful CPR BLS Certification.
• Entry level RN CAP2 with 2 years nephrology nursing experience.

Responsibilities

• Performs patient data collection pre-, during-, and post-hemodialysis and documents in medical record.
• Adjusts treatment plan as indicated; notifies supervisor or physician as needed.
• Educates patients and families about care plans and dialysis options.
• Directs daily delivery of safe hemodialysis therapy in collaboration with care staff.
• Administers prescribed medications and documents effectiveness.
• Participates in emergency response measures when needed.
• Serves as resource and mentor for new staff and other RNs.
• Completes CAP requirements to maintain or advance.
