Job summary

Fresenius Kidney Care seeks an entry-level Registered Nurse for the Clinical Advancement Program (CAP). The role provides and coordinates clinically competent care for an assigned group of patients, including assessment, planning, intervention and evaluation, with possible delegation to RNs, LVN/LPNs and Patient Care Technicians.

As part of the End Stage Kidney Disease team, the CAP RN participates in teaching, leadership functions, and quality improvement activities that enhance patient care

Qualifications

• Graduate of an accredited School of Nursing.
• Current state licensure.
• CPR BLS certification.

Responsibilities

• Performs all essential functions under the direction of the Supervisor with guidance from the Clinical Educator, Preceptor or in collaboration with other RNs.
• Collects and analyzes patient data during the hemodialysis process and documents in the medical record.
• Educates patients and families on learning needs and treatment plans.
• Delivers safe and effective hemodialysis therapy in collaboration with care staff.
• Administers medications as prescribed or per approved algorithms and documents rationale and effectiveness.
• Initiates or assists with emergency response measures.
• Serves as a resource for the health care team and participates in staff training.
• Ensures proper lab collection, processing, and handling; reschedules missed labs.
• Identifies outcomes and updates nursing assessment and care plan with the interdisciplinary team.
• Discusses transplant and treatment modality options with patients.

Education
