Job summary

Fresenius Medical Care is seeking an entry-level Registered Nurse for the Outpatient CAP program. The RN will assess, plan, intervene and evaluate care for an assigned patient group and coordinate tasks with RNs, LVNs/LPNs and Patient Care Technicians.

As a member of the End Stage Kidney Disease care team, the nurse participates in decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

Qualifications

• RN with nursing degree and valid licensure.
• Experience educating patients and families about care plans.
• CAP requirements must be completed to advance in the program.

Responsibilities

• Perform assessment, planning, intervention and evaluation for assigned patients.
• Document data and adjust treatment plans with oversight as needed.
• Coordinate care with RNs, LVN/LPNs and Patient Care Technicians.
