Job summary

Socket.dev is seeking a Care Management Coordinator to support outpatient care management by coordinating patient care services, facilitating communication between patients and providers, and ensuring timely access to healthcare and community resources.

The role collaborates with providers, nurses, social workers, case managers, and interdisciplinary teams to improve patient outcomes, close care gaps, and reduce avoidable ED use and readmissions in outpatient settings.

Qualifications

• Two years of healthcare coordination experience.
• Experience with ambulatory/outpatient care coordination.
• Experience with Medi-Cal or Medicare Advantage in California.

Responsibilities

• Coordinate patient care services and transitions of care.

• Support care planning and adherence to care plans.
