Job summary

Socket.dev is seeking a Care Coordinator to manage the coordination of services for ROWI clients. You will orient clients, arrange transportation, accompany doctor visits, and schedule activities within an interdisciplinary team.

Responsibilities include ensuring compliance with standards, documenting in EMR, and maintaining professional boundaries. The role requires a high school diploma (Bachelor's preferred) and driving at least two years of experience.

Qualifications

• High school diploma required; Bachelor's degree preferred.
• Minimum of two years driving experience required.
• Experience working with kids in a therapeutic setting is preferred.
• Experience with junior high or high school students is preferred.

Responsibilities

• Ensure programs and services are implemented at high standards with quality of care.
• Supervise clients and ensure attendance at assigned groups.
• Facilitate groups and activities as designated by the Facility Manager.
• Conduct client orientation on admission day and follow-up if needed.
• Coordinate transportation schedule and drive clients as needed.
• Document transportation and group activities in EMR within 24 hours.
• Administer and submit documentation in EMR for urine analysis.
• Assist with facility cleanliness and coordinate daily lunch for clients.
