Job summary

Good Shepherd Health Care System is seeking a Care Coordinator to help manage patient care across the continuum. You will collaborate with providers, patients, families, and community resources to develop care plans, coordinate referrals and authorizations, and ensure timely follow-up within an outpatient setting.

Responsibilities

• Care coordination and planning with providers, patients, families, and teams.
• Coordinate referrals, tests, and follow-ups for timely care.
• Document care plans and updates in the EHR.
• Serve as main point of contact for patient questions and services.
• Assist with back-office duties and clinical support when needed.
• Participate in team huddles and quality initiatives.
