Job summary

Qualifications

• Assists in implementing and monitoring individualized patient care plans.
• Performs monthly patient chart reviews to identify care gaps and preventive needs.
• Monitors follow-up appointments, referrals, diagnostics, and care plan goals.
• Coordinates services among PCPs, specialists, hospitals, SNFs, home health agencies, and community resources.
• Conducts telephonic outreach to patients and caregivers as directed.

Responsibilities

• Assist the RN Care Manager in care plan implementation and monitoring.
• Review patient charts monthly to identify gaps and opportunities for intervention.
• Track follow-ups, referrals, tests, and goals to ensure progress.
• Coordinate care across providers and settings for continuity of care.
